Both tk's were a round apart, and looking at the similarity in names, I would say they were in a group. I checked the logs after both tk's, didn't see either one of them come back.

Nature of their offense - tk'ing and leaving. Right in front of me. The two bodies in the screenshot.
Time and date of their offense - 7/3/2013 - This was early Wednesday morning. Forgive me for not having a specific time, but I only just uncovered this, and it was atleast two/three hours before this request.
If needed, what your relationship was to the offense in question. - Left before I could punish.
In the screenshot, you can see the two bodies, and I believe, based off of the name, it was intentional.
