It is generally believed that they lack protein in their diets but many experts believe the DPRK to be exaggerating it's issues with food and medical supplies as to be able to "milk" more aid out of the West.
As for the camps we only have satellite imagery and South Korean Agents pretending to be ex camp guards from the North.

Religion is heavily discouraged and the church and mosque in Pyongyang seems to be symbolic.
