Server sometimes crashes when banning someone.

On the NW_Reddit_Brigade server, sometimes, not consistently, tempbanning someone causes the server to crash. Less consistently, permabanning does the same. Ideas?

Also, is there an error log somewhere that I could use to check these things? I have full access to the server it's hosted on.
