Camera Problems Please Help!!!!

Okay so every now and then, my camera zooms into the head of my character, making it almost impossible to see anything else. I then may quit the server or die or quit the game, and it is back to normal. I also have tried re-installing the game and DLC. However it has continued to happen. Here are some screenshots for you to better visualize it all:

Press the - key to fix the issue, the + and - use the camera

Yup, and restarting the game should fix it to default.

I don't understand...
On your numberpad, there are 2 buttons, the - and the +. Use them to zoom in/out your camera.

Did the suggestions help? Also might be a plugged in USB controller interfering with your zoom controls. Had this same problem and it was my controller.
