I'm no where near 100% completion yet and I've played 69 hours (I do every sidequest, bounty, etc). And too my knowledge I'm not even that close to the end of the story yet (I'm on the final island though). One of the most immersive games where you start to really think about which choices you should take to get the outcome that is best for your play-style. Though it did annoy me how lots of the choices I made in Witcher 2 had no real affect on Witcher 3.
I won lots of things, I came 2nd and 3rd in lots of things, I guess I did some other shit too........ I'm also an FSE legend, probably most commonly described as a cunt. If the shit I do doesn't make sense in your head, well fuck you because it makes sense in mine.

Which i did, against known and reputed player, some of them considered legend, such as, Mandarin, Ledger, Tiberias, Herishey, Hokej, Troister, Axiom, Evanovic, Stark, Eddie, Jammo, Bagins, Freddie, Python. I didn't had a good relationship with most of them, but i congratulate them for what they did, and i had pleasure facing them.

Oh alright. But whenever you complete the game is it possible to travel to all these places?

About the combat, do you only have to fight these monsters or also some Humans?

There is a reason a Witcher carries two swords, one for monsters and one for humans. So both depends which area you are in tbh. In Novigrad you mainly fight humans where as on Skellige I found I fought more monsters. And yes you can travel to any part of the map at any time (though some quests or bounties get cancelled out by others, which is why I like to do all sidequests and bounties before I carry on.

Played like 180 hours by now and finished the game twice (with the first dlc which is awesome and challenging) never seen such an interesting fantasy world with such an in-depth lore. Fuck Elder Scrolls thats casual rpg fantasy for everyone imo. You should also have a look at the books and former games since they are great as well.
